1. The purpose of Orofacial Myofunctional Therapy is to improve the overall health, vitality and well-being of the body through education and orofacial exercises. The Orofacial Myologist, Sherry Harrison-Hughes, does not diagnose diseases or conditions.
2 The Orofacial Myologist, Sherry Harrison-Hughes, is not a licensed Naturopathic Doctor, Medical Physician , or a Speech Language Pathologist.
Sherry Harrison-Hughes is not holding herself out to be a Dental Hygienist in this business, and is not practicing Clinical Dental Hygiene while working at Appalachian Mountain Myofunctional Therapy.
3. As part of the Oral Myofunctional Therapy Services, I may be asked to provide information concerning my physical habits, medical history, moods, energy levels, lifestyle and diet. This information is collected to enable the Orofacial Myologist to: (i) assess orofacial function and form (ii) educate me about the benefits of restoring proper function (iii) recommend habit changes to improve overall well-being.
The Orofacial Myologist, Sherry Harrison-Hughes will hold this information in confidence and will not release or disclose this information to any other person, without my prior consent, except as required by applicable law.
4. If the Orofacial Myologist, Sherry Harrison-Hughes, suspects the existence of disease, disorder or condition, I will be informed of this suspicion. However, I acknowledge this is not a diagnosis or conclusion about the state of my health and that I am directed to promptly consult a licensed Physician, Dentist, ENT, or Naturopath about any suspected problems.
5. Any recommendations I follow for changes in habits are in providing Orofacial Myofunctional Therapy Services to me, the Orofacial Myologist, Sherry Harrison-Hughes, is relying upon the truth, accuracy and completeness of all information I have provided are entirely my responsibility.
